Flu Shot Clinic This Week

You should have received an email about The Nurse Office last week as well as a paper note that came home with students. This is information on the upcoming Flu Shot Clinic and information for completing the forms.

McAnally has Flu Shot Forms in the front office and a form went home with each child at McAnally this week. Please continue to turn into McAnally Intermediate this week. CLICK HERE for flu shot forms.

Aledo ISD will host the Parker County Hospital District during the week of October 5-9 for its annual Flu Shot Clinics. Clinics will begin at 7:30 a.m. at each campus except those designated below. Parents can print consent forms here, or find them at their child’s campus. Only children who have completed forms on the day of the Flu Shot Clinic can receive their flu shot without a parent present.

Monday, Oct. 5 -- 7:30 a.m. Aledo High School/Learning Center (when finished at HS)

Tuesday, Oct. 6 -- 7:30 a.m. Coder Elementary and 7:30 a.m. McCall Elementary

Wednesday, Oct. 7 -- 7:30 a.m. Aledo Middle School and 7:30 a.m. Stuard Elementary

Thursday, Oct. 8 -- 7:30 a.m. Daniel Ninth Grade Center and 7:30 a.m. Vandagriff Elementary

Any parent wishing to be present with their child while he/she receives a flu shot needs to be at their child’s campus between 7:30 a.m. and 8:00 a.m. Clinics will close at each campus once all children with completed forms have received their shots.

-730am-830am – Any parent wanting to be with their child when being vaccinated will need to come at this time; all other staff/students will be vaccinated from 830 until finished

-Consent forms must be filled out correctly – look to make sure you include all health questions, insurance information and signature  - must be present

-All student forms need to be turned in to either the front office or nurse office – All adult forms need to be turned in the day of the clinic. Any child coming with their parent at 730, will need to bring their form with them as well

-Any adults using the clinic that could possibly be pregnant must have a doctor’s note

-Attached is the vaccine information to the forms

-Medicaid and insurance companies will be billed for vaccines; children 18 years and younger with no insurance coverage or underinsured will not be charged and those over 18 years with no insurance can pay $20 or if there is insurance, insurance can be billed

-All will be given proof of shots to take home
